NOTICE: The administrative violations inquiry shows both pending and final charges against licensees and permittees. Unless the status is closed and the disposition column reflects the imposition of an administrative punishment, the violations are pending allegations only and no final determination of the charged violations has been made.

Penalty amounts and suspension dates are not currently available for cases older than two years.
